Health Facilitator
EDUCATIONAL QUALIFICATIONS:
- Registered Nurse with college degree in nursing (masters, bachelors, or associates degree)
- Specialization in pediatrics and public health
- Experience working with infants, toddlers and their families

JOB RESPONSIBILITIES:
- Facilitates a learning environment that encourages health and safety
- Models and teaches preventative health and safety practices for parents and staff that contribute to wellness and the development of a healthy lifestyle
- Develops a system for tracking immunizations and well childcare status of children.
- Facilitates screening of vision and hearing of all infants and toddlers
- Incorporates health and safety information into the parent education groups
- Consults with staff and parents who have concerns about the health of individual children or family members and facilitates appropriate referrals
- Assists parents and staff in identifying appropriate health goals for the family partnership agreements
- Acts as a liaison for children and families with community health services
- Develops and refines curriculum materials to be shared with other that reflect best practices
