Health Facilitator

EDUCATIONAL QUALIFICATIONS:

• Registered Nurse with college degree in nursing (masters, bachelors, or associates degree)
• Specialization in pediatrics and public health
• Experience working with infants, toddlers and their families

JOB RESPONSIBILITIES:

• Facilitates a learning environment that encourages health and safety
• Models and teaches preventative health and safety practices for parents and staff that contribute to wellness and the development of a healthy lifestyle
• Develops a system for tracking immunizations and well childcare status of children.
• Facilitates screening of vision and hearing of all infants and toddlers
• Incorporates health and safety information into the parent education groups
• Consults with staff and parents who have concerns about the health of individual children or family members and facilitates appropriate referrals
• Assists parents and staff in identifying appropriate health goals for the family partnership agreements
• Acts as a liaison for children and families with community health services
• Develops and refines curriculum materials to be shared with other that reflect best practices
